This is to inform you that effective immediately, beginning May 18, 2015 through June 30, 2015 any client who is chronically homeless, should be referred to People Assisting The Homeless (PATH) agency to apply for housing assistance. It should be noted that the PATH application requires a VI-SPDAT score. If the household is a family or a single individual who is TAY a VI-SPDAT is not required. However, write “TAY” or “Family” in the space instead of the VI-SPDAT score. Submission of incomplete PATH applications will not be processed by PATH. PATH staff will contact Housing Policy & Development (HP&D) to verify that the applicant is a DMH client. See attached – PATH application and PATH staff contact information. Please direct any questions regarding the eligibility criteria and application process to PATH staff. Do not contact HP&D regarding questions involving PATH’s application process or procedures.
